New Patients

Upon your first visit, we want you to feel prepared, informed, and at peace with as few unknowns as possible. We want you to trust you are making the best decision for you or your loved one’s body, as well as for your personal life circumstances, before walking through our doors.

Your First Visit

Your first visit with us may be for an evaluation during an office visit, electromyography diagnostic testing, or for an x-ray guided injection.

Regardless of your first appointment type, we require all new patients to provide the following:

  • valid, current photo identification
  • a copy of your insurance card(s)
  • completed, necessary paperwork
  • Arrive 30 minutes prior to your appointment time to complete the registration process

Co-pays are collected for all appointments. If you are unable to provide your co-pay upon check-in, please call to reschedule your appointment

In an effort to expedite care for all of our patients, we require at least a 24-hour notice for all reschedules.

Any appointment canceled with less than a 24-hour notice will be charged a fee depending on the type of appointment.

  • A $50 fee will also be charged if you should not show for your office or EMG appointment
  • A $100 fee will be charged if you should not show for an injection appointment

What about insurance?

At PMC, your insurance should be accepted UNLESS it is one of the following:

We do NOT accept:

    Including family planning, emergency, and methodist, maternity
    Including Advantage Health Solutions and United Healthcare-Navigate, UHC Nexus, Priority Health HMO, Jericho CareSource, Freemarket Admin, Blackhawk Claims Services PHCS. We also do not accept “Charity Care” through Parkview.
    Non-PHCS / Multiplan Priority Health.

We do have several insurance plans we need authorization for to apply. Authorization must be received prior to your appointment.

These include:

  • BCBS-MI: Blue Care Network
  • IU Health Plan Medicare Advantage (as long as there is an in-network physician referral).

Fort Wayne Location

In 2019, we relocated to our current building at 7201 Engle Road in Fort Wayne. Parking is easily accessible immediately outside the entrance to our building. The larger, more spacious waiting room makes it more comfortable for patients. We have expanded the number of exam rooms and procedure rooms, allowing us to treat more patients in a timely manner. Services such as Mindset PrTMS, Midwest Concussion Clinic, and Regenerative Medicine are now available.

Our Angola office is located inside the Cameron Hospital Medical Building, 306 E Maumee Street, Suite 103. Helping us serve the northeastern portion of Indiana, southeast corner of Michigan, and northwest communities of Ohio.

Referred Patients

If you have been referred to our office from another provider only to have an injection, we look forward to taking care of you!

Please understand we actively work on processing the procedure referral as soon as we receive it. Our office expedites creating your chart and completing benefits and prior authorization procedures BEFORE we call you to schedule. For this reason, please allow us five business days to complete this process. If you do not hear from us after the five business days, please contact us at 260-432-1800, option 6, to check on the status of scheduling your procedure.
